IMPORTANT NOTICE – Dr. Zumpano Retirement December, 2025

Dear Patient,
I hope this letter finds you in good health and high spirits. I am writing to inform you of an important transition in my professional journey. After over four decades devoted to the practice of medicine, I have made the decision to retire at the end of this year, 2025.

It has been a privilege and an honor to care for you and your families. The trust you have placed in me and the relationships we have built are treasures I will carry with me into this next chapter of my life. While I look forward to the opportunities retirement will bring, I want to ensure that this change is as smooth and seamless for you as possible.

You have a couple of options moving forward regarding your primary care provider (PCP):

1. Elect to Change PCP: If you wish to choose a new PCP now, you are welcome to select from any available PCP within the organization who has open panels. This allows you to transition to a provider of your choice at a time that works best for you.

2. Reassignment Later This Year: If you prefer not to make a selection at this time, the organization will assist in reassigning you to a new PCP later this year. This reassignment will be done thoughtfully to ensure you are matched with a qualified provider to continue meeting your healthcare needs.
Our administrative team is here to guide you through this process and answer any questions you may have. Please feel free to contact the office for assistance in reviewing available PCPs or for additional details about the reassignment process. As my retirement date approaches, I will continue to provide care and support to you to the best of my ability. Thank you for allowing me to be a part of your healthcare journey for all these years.

Wishing you continued health and happiness,
Warm regards,
Antonio V. Zumpano, M.D.
